​Needs Assessment Information

Immediately upon receiving an inquiry regarding senior housing or senior care we will respond in a timely manner to have an initial assessment during the critical hours, if needed we will place a caregiver in the home if applicable. We will also schedule a time to meet to conduct a full assessment. The goal of this needs assessment is to get a better understanding of each person.

Clients will be professionally evaluated to identify which level of care would benefit them most, and to understand the particular needs and concerns of each family. Many aspects are considered to make a successful and appropriate  care transition including but not limited to:
